Enterprises with AI context layers report agent failures at more than twice the rate of those without one

A VB Pulse July 2026 survey of 101 enterprises reveals that companies using governed context layers report AI agent failures at more than twice the rate of those without them. Currently, 68% of enterprises trace confident but incorrect AI answers to inconsistent or missing business context, an increase from 57% in June. Additionally, recurring failures rose to 37%. These findings highlight the significant complexity involved in integrating context layers to mitigate AI hallucinations within enterprise environments.
